How Do I...?

This form can be found by:

1. Logging in to FMH
2. Make sure you are in the correct reinsurance year
3. On the LEFT side of the screen - Click on Policy Management
4. Click on MPCI Application
5. In the TOP RIGHT corner click on PRINT 
6. Choose MPCI Blank Forms under "Report Type:"
7. In the box below, choose Application/Change/Cancel and Transfer Form
8. Choose the appropriate STATE to the RIGHT
9. Also select Farmers Mutual Hail Insurance Company of Iowa for "Policy Issuing Company:"
10. Now click on the PRINT button on the RIGHT side of "Report Options"

 This form can be found by:

1. Logging in to FMH
2. Make sure you are in the correct reinsurance year
3. On the LEFT side of the screen - Click on Policy Management
4. Click on Standalone Crop Hail
5. In the TOP RIGHT corner click on PRINT 
6. Choose Standalone Blank Forms under "Report Type:"
7. In the box below, choose Standalone Crop Hail Application/Amendment
8. Choose the appropriate STATE to the RIGHT
9. Also select Farmers Mutual Hail Insurance Company of Iowa for "Policy Issuing Company:"
10. Now click on the PRINT button on the RIGHT side of "Report Options"

This form can be found by: 

1. Logging in to NAU (Use the correct reinsurance year)
2. Click on RESOURCES and choose FORMS in the menu at the top of the screen (blue background)
3. To make your search go quicker:  
   1. Click on All Policies and Plans - choose MPCI 
   2. Click on Form Categories - choose Sales Closing
4. Choose Application/Transfer/Change/Cancel (ATCC)
5. A new tab will open with the form.  You can print the form from that screen.

2. Click on RESOURCES and choose FORMS in the menu at the top of the screen (blue background)
3. To make your search go quicker:  
   1. Click on All Policies and Plans - choose Crop-Hail
   2. Click on Form Categories - choose Sales Closing
4. Choose   EASYHail Application/Change Form (EZHA)
5. A new tab will open with the form.  You can print the form from that screen.
