How Do I...?
Print a New Blank MPCI Application - FMH
This form can be found by:
- Logging in to FMH
- Make sure you are in the correct reinsurance year
- On the LEFT side of the screen - Click on Policy Management
- Click on MPCI Application
- In the TOP RIGHT corner click on PRINT
- Choose MPCI Blank Forms under "Report Type:"
- In the box below, choose Application/Change/Cancel and Transfer Form
- Choose the appropriate STATE to the RIGHT
- Also select Farmers Mutual Hail Insurance Company of Iowa for "Policy Issuing Company:"
- Now click on the PRINT button on the RIGHT side of "Report Options"
Print a New Blank Standalone Hail Application - FMH
This form can be found by:
- Logging in to FMH
- Make sure you are in the correct reinsurance year
- On the LEFT side of the screen - Click on Policy Management
- Click on Standalone Crop Hail
- In the TOP RIGHT corner click on PRINT
- Choose Standalone Blank Forms under "Report Type:"
- In the box below, choose Standalone Crop Hail Application/Amendment
- Choose the appropriate STATE to the RIGHT
- Also select Farmers Mutual Hail Insurance Company of Iowa for "Policy Issuing Company:"
- Now click on the PRINT button on the RIGHT side of "Report Options"
Print a New Blank MPCI Application - Rain & Hail
This form can be found by:
- Logging in to Rain & Hail
- Make sure you are in the correct reinsurance year
- Click on FORMS in the menu at the top of the screen (green background)
- Click on MPCI Crop Forms (You will need to scroll down - it is towards the bottom)
- Choose MPCI Application/Cancellation/Transfer/Policy Change Reporting Form | RH-5200
- A new tab will open with the form. You can print the form from that screen.
Print a New Blank Standalone Hail Application - Rain & Hail
This form can be found by:
- Logging in to Rain & Hail
- Make sure you are in the correct reinsurance year
- Click on FORMS in the menu at the top of the screen (green background)
- Click on Stand-Alone Forms and Policy Applications
(You will need to scroll down - it is at the bottom) - Choose Crop-Hail Application | RH-655
- A new tab will open with the form. You can print the form from that screen.
Print a New Blank MPCI Application - NAU
This form can be found by:
- Logging in to NAU (Use the correct reinsurance year)
- Click on RESOURCES and choose FORMS in the menu at the top of the screen (blue background)
- To make your search go quicker:
- Click on All Policies and Plans - choose MPCI
- Click on Form Categories - choose Sales Closing
- Choose Application/Transfer/Change/Cancel (ATCC)
- A new tab will open with the form. You can print the form from that screen.
Print a New Blank Standalone Hail Application - NAU
This form can be found by:
- Logging in to NAU (Use the correct reinsurance year)
- Click on RESOURCES and choose FORMS in the menu at the top of the screen (blue background)
- To make your search go quicker:
- Click on All Policies and Plans - choose Crop-Hail
- Click on Form Categories - choose Sales Closing
- Choose EASYHail Application/Change Form (EZHA)
- A new tab will open with the form. You can print the form from that screen.
